Folio 1: case no 1, Samuel Drew, aged 29, Marine; taken ill at Island of Cobras; sick or hurt, contused wound of left leg, when at work at the Naval store on the Island assisting in landing stores from HMS Industry his left leg got bruised between two casks of salt beef; put on sick list, 5 February 1859, discharged 27 April 1859 to duty.

Folios 2-4: case no 2, Antonio Levy, aged 21, Ordinary Seaman; taken ill at Rio de Janeiro; sick or hurt, remittent fever, a native of Italy, stated that when away during the fore noon on duty, was attacked with a severe frontal headache which increased when he came on board and followed by shivering - pain in lumbar region; put on sick list, 10 February 1859, died 14 February 1859 at 1:30 pm.

Folios 4-6: case no 3, Thomas Berry, aged 28, Supernumerrary Able Seaman belonged to HMS Spy; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 10 February 1859, discharged 20 February 1859 to duty.

Folio 6: case no 4, John Sand, aged 36, Quarter Master; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 12 February 1859, discharged 17 February 1859 to duty.

Folios 6-7: case no 5, John Daly, aged 19, Ordinary Seaman; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 1 March 1859, discharged 25 March 1859 to duty.

Folios 7-9: case no 6, Thomas Connor, aged 48, Ship's Cook; taken ill at Rio de Janeiro; sick or hurt, remittent fever, he joined this ship from HMS Industry, complained of chills, headache, vomiting and purging; put on sick list, 27 March 1859, discharged 18 April 1859 to duty.

Folios 9-10: case no 7, Henry Johns, aged 30, Caulker; taken ill on board HMS Siren; sick or hurt, remittent fever, was sent on board HMS Siren to assist in caulking, on returning to this ship found that his hammock was wet and blankets damp, subsequently he complained of shivering, headache, pain in the back and general debility; put on sick list, 1 April 1859, discharged 27 April 1859 to duty.

Folios 10-11: case no 8, George Willerley, aged 18, Boy 1st Class; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 4 April 1859, discharged 22 April 1859 to duty.

Folios 11-13: case no 9, Mr Humphrey Brett, aged 21, Clerk; taken ill at Island of Cobras; sick or hurt, remittent fever, according to the Surgeon this patient after a hard day's work in the office which was very wet and damp went to bed and complained of feeling languid and restless, on the 6 April had chills, headache, pains in the limbs and legs, the Assistant Surgeon Mr Skene went on shore and ordered him a warm bath, he reported to the Surgeon and accordingly the Surgeon recommended that the patient be removed from this ship; put on sick list, 5 April 1859, died 10 April 1859 at 2:30.

Folios 13-14: case no 10, John Ashmore, aged 18, Boy 1st Class; taken ill at Rio de Janeiro Harbour; sick or hurt, remittent fever, the patient joined this ship on 9 July 1858, he left England in an Brazilian Gun boat, complained of cold, frontal headache, eyes painful on pressure; put on sick list, 9 April 1859, discharged 24 April 1859 to duty.

Folios 14-15: case no 11, Mr John Retallick, aged 34, Paymaster; taken ill at Island of Cobras; sick or hurt, remittent fever; put on sick list, 8 April 1859, discharged 21 April 1859 to duty.

Folios 15-16: case no 12, John Woodward, aged 18, Boy; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 18 April 1859, discharged 27 April 1859 to duty.

Folios 16-17: case no 13, Samuel Thain, aged 22, Able Seaman; taken ill at Island of Cobras; sick or hurt, remittent fever; put on sick list, 21 April 1859, discharged 20 May 1859 to duty.

Folios 17-18: case no 14, John Thonrington, aged 20, Ordinary Seaman; taken ill at Island of Cobras; sick or hurt, remittent fever; put on sick list, 30 April 1859, discharged 16 May 1859 to duty.

Folios 18-19: case no 15, Edward Porter, aged 17, Boy 2nd Class; taken ill at Rio de Janeiro Harbour; sick or hurt, remittent fever, according to the Surgeon this boy came on board this ship on 2 June from HMS Cumberland, he was told when on board that ship that he was going to the Madagascar he cried bitterly and as he was coming over this ship's side and said 'I am coming here to be measured', once on board he was unable to walk on account of a severe scald on his left foot, therefore he was for the most part in the sick bay. The Surgeon stated that at this time the fever season had terminated, but however on the 12 June the patient complained of headache, pain in back and general uneasiness, pulse 140, skin very hot and dry, bowels confined and eyes heavy; put on sick list, 12 June 1859, died 16 June 1859 at 2 am.

Folios 19-21: case no 16, Edward Morgan Russell, aged 18, Boy 2nd Class; taken ill at Rio de Janeiro; sick or hurt, remittent fever; put on sick list, 7 June 1859, died 17 June 1859 at 4 am.

Folios 21-23: case no 17, William Moore, aged 26, Marine; sick or hurt, remittent fever, according to the Surgeon, this man came from HMS Harrier and joined this ship on 7 July, he formerly served on the Coast of Africa and West Indies Stations - some of the men on board this ship told him that 'should he join the Madagascar he would be sure to have fever' he assured them by saying that 'he had had fever in the West Indies and that 'he was not the least afraid of fever'; put on sick list, 19 July 1859, discharged 24 August 1859 to duty.

Folios 23-25: case no 18, Samuel Hallett, aged 16, Boy 2nd Class; sick or hurt, remittent fever, on 1 August he was very restless during the night, skin increased in heat, tongue moist and red, eyes and trunk yellow, urine dark and albuminous - the limbs had assumed a more dark blue colour all over, much swollen and painful to the touch, at this time the Surgeon was unwell himself, he recommened Mr Skene Assistant Surgeon to call on Dr [Azerede?] a medical officer at [Jura Juba Hospital?] (Yellow Fever Hospital) who recommened that for the sake of the other patients, this boy should be removed to the Yellow Fever Hospital; put on sick list, 25 July 1859, died 2 August 1859 at 2 am.

Folio 26: case no 19, Mr J W Lowe, aged 28, Master; sick or hurt, stricture of urethra, he served on board this ship since October 1855, during the last hot season his health considerably failed him, suffering from dyspepsia, obstinate diarrhoea - irrigular rigors from stricture of urethra, the surgeon recommened him as fit subject for invalided, he was accordingly surveyed; put on sick list, 3 May 1859, discharged 26 May 1859.

Folios 26-27: case no 20, William Ash, aged 43, Ward Room's Steward; sick or hurt, gout and rheumatism combined; put on sick list, 25 March 1859, discharged 9 April 1859 to duty, invalided 26 June 1859.

Folios 27-28: case no 21, E M Leycester, aged 42, Commander of HMS Madagascar; sick or hurt, [ramollissement?], complained of giddeness, a sense of fulness in the back part of the head attended with nervous tremor, and in general a want of vital active, and any duty that called into action his brain brought on the symptoms; put on sick list, 16 August 1859, invalided 6 September 1859.

Folio 28: case no 22, Michael Cowling, aged 20, Ward Room's Steward; sick or hurt, hip joint disease; put on sick list, 15 October 1859, invalided 4 November 1859.

Folio 29: case no 23, John Hawkesley, aged 30, Corporal Marine; sick or hurt, phthisis pulmonalis; put on sick list, 25 November 1859, invalided 29 December 1859.

Folio 30: case no 24, Frederick Roberts, aged 23, Able Seaman; sick or hurt, abscess; put on sick list, 26 October 1859, discharged 12 December 1859 to HMS Siren.
