Folios 20-24. Letter from Thomas Marriott, Clerk to the Guardians of the Southwell Union, to the Poor Law Commission, asking for a decision re a 'medical charge' of £2 by Mr HR Smith, Surgeon, for attendance on Sarah Mayfield. There are enclosed letters from Mr Smith and Mr Francis Gee, overseer at Stoke.
Apparently, Sarah Mayfield sent for Mr Smith 'on her own account without any order from the overseer'. Sarah Mayfield, 'a poor woman' had been pregnant with twins and was dangerously ill after a botched delivery by the midwife; she was later sent to hospital in Nottingham and treated by Mr Smith on her return. Gee states that the family's 'club' [friendly society] should meet the cost.
