Folios 1-2: John Tobin, aged 38, emigrant; disease or hurt, acute lumbago then severe rheumatism. Put on sick list, 12 May 1825 in the Cove of Cork. Discharged 25 May 1825 cured.

Folios 2-4: Michael Quinilan, aged 10; disease or hurt, fever, severe headache, accompanied with a diminuation of sight, face flushed, eyes slightly inflamed. Put on sick list, 8 May 1825 in Cork. Date of discharge [not stated].

Folios 4-5: Robert Cunningham, aged 24; disease or hurt, violent vomiting and purging. Put on sick list, 10 May 1825 at sea. Discharged, May 1825 [day not stated].

Folios 5-7: John Kennedy, aged 28; disease or hurt, severe tormina, tenesmus, want of appetite, general pains and soreness with inclination to vomit. Put on sick list, 21 May 1825 at sea. Discharged 30 May 1825 cured.

Folios 7-8: James Handlow, aged 22; disease or hurt, acute pain of the thorax and in the right side, severe cough, difficult expectoration attended with dyspnea. Put on sick list, 28 May 1825. Date of discharge, not stated.

Folios 8-9: John Sullivan, aged 27; disease or hurt, fell from the main ladder to the lower deck dislocating right arm. Put on sick list, 30 May 1825. Discharged 10 June 1825 cured.

Folios 9-12: Maurice Gibbons, aged 74; disease or hurt, owing to a heavy gale of wind on the banks of Newfoundland, in the middle of the night, was thrown out of his sleeping berth. Complains of pain and difficulty of breathing. Put on sick list, 31 May 1825. Died 12 July 1825 within one days journey of Prescott.

Folios 13-15: Eugene Gardiner, aged 22; disease or hurt, acute rheumatism. Put on sick list, 30 May 1825. Discharged 12 July 1825 cured.

Folios 15-17: Mary Collins, aged 16; disease or hurt, soreness and tightness of the throat attended with thirst, headache, hoarse loud cough, difficulty of degution and respiration. Put on sick list, 4 June 1825. Discharged 19 July 1825 cured.

Folios 17-18: Bridget Shannahan, aged 30; disease or hurt, violent and pungent pains in the epigastric regions with vomiting and symptomatic fever with a small frequent and hard pulse accomapined with a feeling of great debility and some purging. Put on sick list, 4 June 1825 at the gulf of St Lawrence. Discharged 14 June 1825 cured.

Folios 18-20: Mary Sullivan, aged 18; disease or hurt, severe tormina, tenesmus accompanied with frequent [?] mixed with blood, want of appetite, general pains and soreness. Put on sick list, 7 June 1825 at river of St Lawrence. Discharged 22 June 1825 cured.

Folios 20-21: Mary Kean, aged 21; disease or hurt, seized with violent pain in the back and loins with sickness and desire to throw up. Put on sick list, 13 June 1825. Discharged 25 June 1825 cured.

Folios 21-23: John Falvey, aged 16 months; disease or hurt, diarrhoea. Put on sick list, 17 June 1825 at River St Lawrence. Discharged 4 July 1825 cured.
