  • Index preceding the register on unnumbered pages
  • Register on pages 1 to 336. Pages 217 to 218 missing, probably removed whilst register was in use. Pages 263 to 265 missing, stuck together. Pages 271 to 272 missing, probably removed whilst register was in use.
