Ailments listed: pain of buttcks, abdomen and right thigh, abdomen distended and rigid, diarrhoea. Operation, incision made in right buttock, fluid and gas drained. Abscess developed in buttock. Condition deteriorated, discharge persisted. Patient died 29/04/1915. Clinical chart attached.
